This antibody recognises proteins of 80-200 kDa, identified as different members of CEA family. CEA is synthesised during development in the fetal gut and is re-expressed in increased amounts in intestinal carcinomas and several other tumors. This mAb does not react with nonspecific cross-reacting antigen (NCA) and with human polymorphonuclear leucocytes. It shows no reaction with a variety of normal tissues and is suitable for staining of formalin/paraffin tissues. CEA is not found in benign glands, stroma, or malignant prostatic cells. Antibody to CEA is useful in detecting early foci of gastric carcinoma and in distinguishing pulmonary adenocarcinomas (60-70% are CEA+) from pleural mesotheliomas (rarely or weakly CEA+). Anti-CEA positivity is seen in adenocarcinomas from the lung, colon, stomach, esophagus, pancreas, gallbadder, urachus, salivary gland, ovary, and endocervix.

In humans, multiple isoforms of Cytokeratin 6 (6A-6F), encoded by several highly homologous genes, have distinct tissue expression patterns, and Cytokeratin 6A is the dominant form in epithelial tissue. The gene encoding human Cytokeratin 6A maps to chromosome 12q13, and mutations in this gene are linked to several inheritable hair and skin pathologies. Keratins 6 and 16 are expressed in keratinocytes, which are undergoing rapid turnover in the suprabasal region (also known as hyper-proliferation-related keratins). Keratin 6 is found in hair follicles, suprabasal cells of a variety of internal stratified epithelia, in epidermis, in both normal and hyper-proliferative situations. Epidermal injury results in activation of keratinocytes, which express CK6 and CK16. CK6 is strongly expressed in about 75% of head and neck squamous cell carcinomas. Expression of CK6 is particularly associated with differentiation.

These markers may be useful in identification of these organelles in cells, tissues, and biochemical preparations. It recognises an antigen associated with the Golgi complex in human cells only. It can be used to stain the Golgi complex in cell or tissue preparations and can be used as a Golgi marker in subcellular fractions. It produces a diffuse staining pattern of the Golgi zone in normal and malignant cells and may be used to stain Golgi complex of cells in frozen tissue sections. It can also be used with paraformaldehyde fixed frozen tissue or cell preparations. This mAb is an excellent marker for human cells in xenographic model research. This antibody recognises an oncofetal antigen of 220 kDa, identified as a tumor-associated glycoprotein (TAG-72) with properties of a mucin. This antibody defines the mucin-carried sialylated-Tn epitope. TAG-72 is usually expressed by adenocarcinomas, but is negative in mesotheliomas. Studies have reported that this antibody has 80% sensitivity and 93% specificity for pulmonary adenocarcinoma. Therefore, TAG-72 is a useful marker to distinguish between mesothelioma and adenocarcinoma. However, false positive reactions can occur so results must be interpreted with the utmost caution. This antibody may be useful in the differentiation of non-small cell carcinomas from small cell carcinomas of the lung. The combined use of TAG-72 Ab with a GCDFP-15 Ab is valuable in the diagnosis of apocrine carcinoma.

Reacts with human CD59, a 20 kDa glycosyl phosphatidyl-inositol (GPI)-anchored cell surface protein. CD59 regulates complement-mediated cell lysis, and it is involved in lymphocyte signal transduction. This protein is a potent inhibitor of the complement membrane attack complex, whereby it binds complement C8 and/or C9 during the assembly of this complex, thereby inhibiting the incorporation of multiple copies of C9 into the complex, which is necessary for osmolytic pore formation. CD59 is widely distributed on cells in all tissues. It inhibits formation of MAC, thus protecting cells from complement-mediated lysis. The expression of CD59 on erythrocytes is important for their survival. Genetic defects in GPI-anchor attachment, that cause a reduction or loss of CD59 and CD55 on erythrocytes produce the symptoms of the disease paroxysmal hemoglobinuria (PNH). It is useful for study on GPI-anchored proteins, PNH and CD59 functions.

This mAb reacts only with the intact-HCG and not with either free alpha- or free beta-chain of HCG. HCG is a glycoprotein and is composed of two non-identical, non-covalently linked polypeptide chains designated as the alpha and beta subunits. The alpha subunit is identical to that of thyroid stimulating hormone (TSH), follicle stimulating hormone (FSH), and luteinizing hormone (LH). HCG is secreted in large quantities by normal trophoblasts. It is present only in trace amounts in non-pregnant urine and sera but rises sharply during pregnancy. HCG mAb detects cells and tumors of trophoblastic origin such as choriocarcinoma. Large cell carcinoma and adenocarcinoma of the lung demonstrate anti-hCG positivity in 90% and 60% of cases respectively. 20% of lung squamous cell carcinomas are positive. HCG expression by non-trophoblastic tumors may indicate aggressive behavior.

It is expressed in epithelial cells of the thyroid gland and the lung. Nuclei from liver, stomach, pancreas, small intestine, colon, kidney, breast, skin, testes, pituitary, prostate, and adrenal glands are unreactive. Anti-NKX2.1 is useful in differentiating primary adenocarcinoma of the lung from metastatic carcinomas originating in the breast, mediastinal germ cell tumors, and malignant mesothelioma. It can also be used to differentiate small cell lung carcinoma from lymphoid infiltrates. Loss of NKX2.1 expression in non-small cell lung carcinoma has been associated with aggressive behavior of such neoplasms. Reactivity is also seen in thyroid malignancies.

Actin is a major component of the cytoskeleton and is present in most cell types. This mAb is highly specific to actin from smooth muscles. Its epitope lies in the first four N-terminal amino acids. This mAb does not stain cardiac or skeletal muscle; however, it does stain myofibroblasts and myoepithelial cells. This antibody could be used together with anti-muscle specific actin and myogenin in making a diagnosis of smooth muscle and skeletal muscle tumors. In most cases of rhabdomyosarcoma, this antibody yields negative results whereas anti-muscle specific actin and myogenin are positive. Leiomyosarcomas are positive only with anti-muscle specific actin and anti-smooth muscle actin and are negative with anti-myogenin.

Reacts with a 70 kDa membrane-bound isozyme (Regan and Nagao type) of placental Alkaline Phosphatase (PLAP) occurring in the placenta during the 3rd trimester of gestation. It is highly specific for PLAP and shows no cross-reaction with other isozymes of alkaline phosphatase. Anti-PLAP reacts with germ cell tumors and can discriminate between these and other neoplasms. Somatic neoplasms e.g. breast, gastrointestinal, prostatic, and urinary cancers may also immunoreact with antibodies to PLAP. Anti-PLAP positivity in conjunction with anti-keratin negativity favors seminoma over carcinoma. Germ cell tumors are usually anti-keratin positive, but they regularly fail to stain with anti-EMA, whereas most carcinomas stain with anti-EMA. Anti-PLAP has been useful in the diagnosis of gestational trophoblastic disease.

Estrogen receptors (ER) are members of the steroid/thyroid hormone receptor superfamily of ligand-activated transcription factors. CD30 is synthesised as a 90 kDa precursor, which is processed in the Golgi complex into a membrane-bound phosphorylated mature 105/120 kDa glycoprotein. In Hodgkin’s disease, CD30 antigen is expressed by mononuclear-Hodgkin and multinucleated Reed-Sternberg cells. It is also expressed by the tumor cells of a majority of anaplastic large cell lymphomas as well as by a varying proportion of activated T and B cells. This antibody distinguishes large cell lymphomas derived from activated lymphoid cells from histiocytic malignancies and lymphomas derived from resting and precursor lymphoid cells or from anaplastic carcinomas. About one third of the CD30 positive lymphomas lack the leukocyte common antigen (CD45).
